Thanks for your thoughts and question. An excellent one to explore.
Clearly no one knows the answer. My speculation is ‘conventional wisdom’ and SCOTUS pundits may be surprised by the eventual ruling.
None-the-less, there is no alternative model than managed care 2.0 aka ‘accountable care’ movement or ACOs in particular. As far as the Medicare market is concerned, there is a direct link to ACA and the Medicare Shared Savings Program and it’s derivatives (advanced payment, bundling, Pioneers, etc). Yet, the commercial market is less tethered to the decision. They are moving in that direction anyway exploring ‘accountable care arrangements’ via contractual engagement, less driven by CFR’s of specific acts per se.
Docs need be clear that the market is changing whether ACA stays in tact or not. At this point it’s about staying connected to the conversation and vetting local initiatives. Docs have choices and are really in the driver’s seat though most don’t see it that way.

I am a midsize revenue cycle management company (morphed from a traditional billing company), been doing well but now a lot of my customers cannot manage being independent and are either closing or becoming hospital owned.
I have to change our traditional business model, ACO seems to be the future but don’t know what could be our play? Our customers are encouraging us to bring them a Integrated Solution whereby they can enter into a IPA sort of arrangement and then evolve into a ACO, is this the right way? Is there a body or group I can get direct help/consultancy from? We lack the knowledge of how to integrate this all together

Thanks for the comment, thoughtful reflection and your very timely question.
While there are no answers that will apply in every situation, finding realistic bridges or enabling gateways to a somewhat unsure future is a smart move. My take is your on the right path. Finding an integration opportunity that can attract, engage and coalesce like minded and mission oriented physicians is gravitational pull that should play. ACOs will sport many of the features of v1.0 generations of IPA, their MSOs and even institutional partners whether members in an entity or not, sprinkled with new, improved and rather robust SaaS platforms, from EHRs to scheduling.

As Masters says, there are no universal answers. We (our multi-specialty physician cooperative) is forging ahead along the same path as Masters suggests; by pooling resources, centralizing some management functions, etc. and otherwise simply benefiting from the economy of scale of our integrated group and the “gravitational pull” of “like-minded and mission-oriented physicians.” This maxim, by itself is deceivingly difficult, at least for me, the only non-physician in the group. Our physicians are being motivated by fear, and that fear manifests differently in every practice; so keeping the gravitational pull is sometimes a daunting and exhausting task. Add to that the conundrum of advising your primary care physicians differently than your specialists, and coping with their respective outlooks for the future.
Our business model looks nothing like the IPAs or MSOs of that bygone era. In fact, it looks nothing like our own business model from two years ago! All I can tell anyone is that we feel better by being together, if only in commiseration. We are dedicated to remaining independent; and none of us have signed the ‘accountable care contracts’ offered thus far. I am advising caution and patience right now, especially for our PCPs. However, come June and November, that may very well change, and we will likely be redefining our business model once again.
I have shared little in this sermon, and like anyone reading this, looking for more answers than I am offering. I am hoping to give my physicians some tangible answers soon. I am open to suggestions.

http://StratCenter.com offers ACO databases. 1 has the information about the ACO itself, and another details all providers that participate in that ACO (about 5-6,000 total records). StratCenter tracks all IDNs, GPOs, Group Practices, and ACOs
