A Profile of ACO’s Selected for July 1, 2012 Start Date
Per CMS Annoucement:
On July 9, 2012, the Centers for Medicare & Medicaid Services (CMS) announced the selection of 89 additional Accountable Care Organizations (ACOs) to participate in the Medicare Shared Saving Program. The selected organizations will take responsibility for coordinating care for nearly 1.2 million beneficiaries in 40 States and Washington, D.C. In total, there now are 154 organizations participating in Medicare shared savings initiatives, serving over 2.4 million Medicare patients across the country.
All ACOs that succeed in reducing the rate of growth in the cost of care while providing high quality care may share in the savings to Medicare. To ensure high quality of care, ACOs will report performance on 33 measures relating to care coordination and patient safety, use of appropriate preventive health services, improved care for at-risk populations, and patient and caregiver experience of care.
Five of the ACOs announced today applied for a version of the program that allows them to earn a higher share of any savings by also being held accountable for a share of any losses if the costs of care for the beneficiaries assigned to them increase.
Participation in an ACO is purely voluntary for providers. Because the Shared Savings Program is part of the original Medicare fee-for-service program, beneficiaries served by these ACOs will continue to have free choice about the care they receive and from whom they seek care, without regard to whether a particular provider or supplier is participating in an ACO.

Seems like a huge problem that some of the most likely ACO type groups aren’t participating in the program. Companies like Kaiser and IHC should be doing this, but for some reason saw a flaw in the program. I don’t think that bodes well for the government’s ACO program.
Could be, time will tell. None-the-less, the horse is out of the barn. Whether we witness ‘federally qualified ACOs’ or their private market derivatives largely unencumbered by Medicare and Medicaid Federal regulation per se, and more or less implementing their local vision of ‘accountable care’ with and without the ‘O’, ACOs are here to stay regardless of AMGA’s hedging position. The Pioneer class will likely demonstrate the value proposition through such determined innovators as Advocate Health, Monarch Healthcare, Brown and Toland, Heritage, Sharp Healthcare, and many other brave leaders. Personally, not too concerned about those who chose to sit on the sidelines. As proferred at the ACO Summit in DC: ‘…pioneers may take the arrows, but settlers get the land.’ For me, mature risk bearing entities who chose to sit on the sidelines may be playing that ‘conservative card’ (kind of ironic) vs. challenging the strategic paradigm, which is rock solid in my view! I agree that we’re headed that direction regardless. I think the IHC and Kaisers of the world see that as well, but I think they don’t see that in the government programs. Makes one wonder what’s different.
One could ask the question, since we’re headed that direction anyway, do we need the govt programs to spur the adoption of ACOs?
Great question! My view, absent DARPA we’d likely never see the internet at it’s current state of evolution and utility if at all. Absent NASA we’d not be as connected as we are today. Government has a role to enable innovation, then get out of the way. A delicate balance that no doubt gets muted if not lost in the idealogical warfare we’re seeing today. Pretty clear to me, absent CMMI we’d not see the inroads both Aetna and United HealthGroup are making in a frenzied market. According to Charles Saunders, MD at the Wireless Life-Sciences Alliance summit in San Diego, in May of this year he had a backlog of 180 provider requests to work with Aetna. It’s working [i.e., the triple aim culture is being infused in healthcare. No more ‘business as usual’ options on the strategic planning matrix. This time is transform or die!].
